When submitting a FM Equipment Request to Auditory Outreach, please ensure the request includes:
1. A completed and signed Auditory Outreach FM Equipment Request form
2. A signed Permission-to-Share form.
3. Student's most recent audiology report.

You will be notified via email when we receive the student's FM Equipment Request and whether the submission is complete, or requires additional information.
