Bus Pass Reprint Request

Please provide the following to request a reprint of your childs bus pass. and send it to 

  • Student Name 
  • School
  • Change of information
  • Phone
  • Replacement Reason:
    • Lost or Broken
    • Stolen or No longer scanning working

*Replacement Cost: There is a 5$ fee for the replacement of cards that have been lost or broken. If a card is no longer scanning please turn it into your driver and they will ensure it is replaced at no cost. If you card has been stolen, please indicate below and the fee will be waived. Thank you.
